You get a first move for each of them, something small enough to do this week without a budget or anyone’s permission. What it will not do is tell you which one to move first, or what it is costing you to leave it. That depends on your site, your team and your week, and it is what the free call at the end is for.

A score is a starting position, not a report card. It measures how much of an ordinary week your design absorbs before it reaches a person. It is also the number that moves the moment you change something structural, which is the whole point of measuring it.
Each scored 0 to 3. The two lowest are where your design is being asked to do the most.
The seven domains score how your operation is built. The Base Line does not score a part of it, it records what an ordinary day feels like right now. It is not added to your Design Score and it does not change your pattern. It is the mark you measure against when you come back.
Here is the part worth sitting with: both of those are design problems, and design problems are the fixable kind. They are not about trying harder, hiring better, or caring more. Something in the structure is asking a person to absorb what the system should be absorbing, and structure is the one thing in your venue you can actually change on purpose.
